No. The first decision is whether the current foundation can be recovered safely. I recommend a rebuild only when continued patching is more expensive or risky.
Not by default. Diagnosis uses logs and a copy, while fixes go to staging first. Production changes happen only after testing and a rollback path are agreed.
If I deliver the Rescue Sprint, the diagnosis fee is deducted from the sprint price. If another team implements the work, the report remains a standalone deliverable.

You can finish with the report or move to separately priced technical care. The sprint does not force a retainer.
